This English version is provided for convenience. The German version is legally binding.

1. Controller

The controller responsible for data processing on this website is:

Henrik Voß
Mentis Uno – psychological assessment
Johannisberger Straße 22, 14197 Berlin, Germany
Email: anfragen@mentis.uno

2. The key points in brief

  • This website sets no cookies and uses no analytics or tracking tools.
  • Fonts and all content are loaded from our own web space. No data is transferred to services such as Google Fonts, map or video providers.
  • The self-assessments are evaluated exclusively in your browser. Your answers are neither stored nor sent to us.
  • We only receive personal data if you write to us via the contact form or by email.

3. Hosting

This website is hosted by statichost.eu, a provider based in Sweden. The servers are located in the European Union. When you visit the website, the host processes technically necessary data so that the pages can be delivered. This includes in particular your IP address. According to its own information, statichost.eu uses the IP address only to deliver the pages and does not store any personal data in server logs.

The legal basis is our legitimate interest in providing the website securely and reliably (Art. 6(1)(f) of the EU General Data Protection Regulation – GDPR, German: DSGVO). A data processing agreement pursuant to Art. 28 GDPR has been concluded with the host.

4. Self-assessments (questionnaires)

The questionnaires on our service pages are evaluated entirely in your browser. Your answers do not leave your device. We neither store nor transmit them, and we do not learn of them. When you reload or close the page, the answers are deleted.

If you click "Request an appointment" after the result, only the topic (for example "ADHD assessment") is pre-filled in the contact form. Your individual answers and your result are not transferred.

5. Contact form and email

If you write to us via the contact form, we process the details you provide there: name, email address, phone number if given, your topic, your preferred time to be reached and your message. We use this data exclusively to handle your enquiry and to get in touch with you.

Your topic may allow conclusions about your health (Art. 9 GDPR). We therefore process your details on the basis of your explicit consent (Art. 6(1)(a) and Art. 9(2)(a) GDPR), which you give by submitting the form. Where the enquiry concerns entering into an assessment or treatment contract, Art. 6(1)(b) GDPR is an additional legal basis. You can withdraw your consent at any time with effect for the future, for example by email to anfragen@mentis.uno.

To transmit the form we use the service Forminit (UXPLUS LTD, London, United Kingdom). Forminit receives the form data and forwards it to us by email. A data processing agreement pursuant to Art. 28 GDPR has been concluded with the provider. According to the provider, the data is stored encrypted in data centres in Ireland, i.e. in the European Union. We have set the storage period at Forminit as short as possible; after that, the data is deleted there automatically. Insofar as the provider accesses the data from the United Kingdom, this is covered by the European Commission's adequacy decision for the United Kingdom (Art. 45 GDPR).

To protect against automated spam submissions, the form contains an invisible field. No cookies are set for this and no services such as reCAPTCHA are used.

If you write to us directly by email, we process your details in the same way. Our mailbox is operated by an email provider with whom a data processing agreement pursuant to Art. 28 GDPR has been concluded. Please note that unencrypted emails could be read by third parties in transit. Therefore, please do not send us detailed health information by email.

6. Storage period

We delete your enquiry as soon as it has been dealt with and no retention obligations apply. If an assessment takes place, the statutory and professional retention periods apply to the documents created in the process.

7. Disclosure to third parties

We do not pass your data on to third parties, except to the service providers named above who act on our behalf, or where we are legally obliged to do so. No automated decision-making or profiling takes place.

8. Your rights

You have the right of access (Art. 15 GDPR), rectification (Art. 16 GDPR), erasure (Art. 17 GDPR), restriction of processing (Art. 18 GDPR), data portability (Art. 20 GDPR) and objection (Art. 21 GDPR). You can withdraw any consent you have given at any time (Art. 7(3) GDPR). Simply contact us at anfragen@mentis.uno.

You also have the right to lodge a complaint with a data protection supervisory authority. The authority responsible for us is the Berlin Commissioner for Data Protection and Freedom of Information (Berliner Beauftragte für Datenschutz und Informationsfreiheit), Alt-Moabit 59–61, 10555 Berlin, Germany.

9. Encryption

This website is delivered exclusively via an encrypted connection (HTTPS). This also applies to submitting the contact form.

10. Changes

We update this privacy policy when the website or the legal requirements change. The version published here applies.
